How we work

GroundGame.Health has developed a business model that brings diverse stakeholders, processes, terminologies, technologies, and payment structures together, to connect people to the community-based organizations and resources they need on the ground, to overcome the barriers that are preventing them leading healthier lives.

Whether a person needs transportation, food, housing, help applying for benefits, or another needs, we have the boots on the ground to support them.

Through human-to-human connections, we build trust through compassion and empathy with each person we engage. That trust enables us to uncover and solve the most important obstacle each person is facing.

1: Population

We begin by identifying the population of individuals who currently have unmet social needs.

2: Integrations

Our bidirectional integration capabilities with customers allows us to exchange information such as referrals, care plans, claims, etc.

3: Implify™

The Implify™ platform intelligently routes information to relevant resources, and documents both the activities performed, and the results achieved.

4: Help

GroundGame.Health staff, along with community-based organizations and/or resource providers, work directly with the people who need help — on the ground.

5: Document

The Implify platform creates a secure, cohort-based structure to assess and capture health-related social needs, documents gaps that are closed, as well as the resources that were ultimately provided.

6: Financial

The Implify platform enables both upstream and downstream billing and payments, through invoices and/or claims.

Our success lies in a comprehensive solution that identifies and tackles barriers, pays for them, and documents every step in our platform — Implify™. Implify is HITRUST certified and seamlessly integrates with other healthcare technology systems, ensuring a smooth and transparent process from assessment to care planning, outcome reporting, and closing social and care gaps.

GroundGame.Health has developed a national network of community-based organizations (CBOs), enabling us to solve complex social needs at a hyperlocal level. Our unique model positively impacts the communities served because we have flowed more than $36M downstream to the CBOs, advancing their missions.

Randy*

Randy, 75, had mobility issues, as his rollator was both too small, and in disrepair. Also, his caregiver was unreliable, and his cell phone was not working — making it impossible to seek help. Our care coordinator intervened, arranging a new rollator prescription, and helping source a new one. She intervened with the care agency on his behalf, and also arranged for a new phone. The support has led to a much better quality of life for Randy.

Jenny*

Jenny is a 58-year-old woman who despite her employment status, struggles from time-to-time keeping food on the table, and paying her utilities during the cold weather months. We helped Jenny sign-up for food support that she qualified for, and connected her with a local resource that provides financial assistance with her utility bills.

José, 65, is diabetic, but was having trouble affording his prescribed medication and testing equipment. As a result, he was not feeling well, with elevated A1c values. With our care coordinator’s help, José was connected to a program to help pay for his medication and testing gear. The coordinator even followed up with José's PCP to see if more help was needed. Today, José’s diabetes is well-controlled.
