ENABLING BETTER HEALTH
Our unique approach brings everything together, removing barriers and enabling people to live their best lives.

About our solutions
At GroundGame.Health, we manage the complex connections between health plans, providers, employers, community-based organizations (CBOs), and other stakeholders to fulfill unmet health-related social and care needs for people across the country. By facilitating personalized, culturally tailored, human-to-human interactions, we make the biggest difference in people's lives.
Our scalable, hyperlocal approach is driven by our proven technology platform and national network of CBOs. Through our unique model, we increase financial capacity within communities to sustain social care delivery, making it easier for health plans and CBOs to fulfill their shared goal of closing the loop on social needs and gaps in care.

How we’ve worked with our partners to make a difference
Real people, real stories.

Larry*
Larry is a 52-year-old man who has high blood pressure and diabetes. He struggles to afford costly GLP-1 and diabetes medications and has difficulty in paying his monthly bills. He didn't realize that he qualified for the financial assistance with utilities that we found for him. We also helped him sign-up for a program to lower the cost of his medications.

Roberta*
Roberta, a single parent of young son, struggled to find stable housing and enough food. When her health plan contacted her about routine exams that she’d missed, her situation came to light, and she was referred to GroundGame.Health. Our care coordinator, Tiara, worked to identify and provide resources to Roberta. She said that Tiara was “giving me a leg up in the game” by ensuring that she now had secure housing and reliable food support.

Chantel*
José, 65, is diabetic, but was having trouble affording his prescribed medication and testing equipment. As a result, he was not feeling well, with elevated A1c values. With our care coordinator’s help, José was connected to a program to help pay for his medication and testing gear. The coordinator even followed up with José's PCP to see if more help was needed. Today, José’s diabetes is well-controlled.
